We are implementing a new way to collect all of the necessary forms for our student athletes. We are using a program called Final Forms. You will need to sign up and register your student(s) for at https://baugo-in.finalforms.com/ This is where we will house the physical form digitally. Please register and then fill out the medical history portion for your doctor. After that portion is complete, you will be able to print the physical. You can bring the physical back to your coach or school when completed. We will then upload the form to the site. All information will be housed there. If you are unable to print, you may get a printed copy from the athletic director or trainer's office. The attached document will tell you how to register. An easy way to get to the site will also be on our website labeled Final Forms to the left hand of the site. Be sure to type in Baugo as it asks for the district, not the school.
